Our comprehensive diabetes education program provides the foundation and support to help people with diabetes navigate these self-care decisions every day. We collaborate with patients and the community to empower and encourage them to achieve their optimal level of health by equipping them with the tools needed to effectively self-manage their diabetes.

Research shows that proper diabetes education improves quality of life, helps prevent long-term complications, and even reduces the risk of mortality*. Let our experienced Certified Diabetes Care and Education Specialists (CDCES) guide you through the large amount of information, technologies, and resources available in this rapidly changing field. Our team includes Registered Nurses, Registered Dietitians, and an Exercise Physiologist.

What We Offer

If you are newly diagnosed with diabetes or newly motivated, you will work with a diabetes educator to create an individualized plan based on your needs. This assessment session takes about an hour. Next, for those living with type 2 diabetes, we offer a two-class series to teach you the basics of living and eating well with diabetes. For those living with type 1 diabetes, we offer two, 2-hour sessions, working with a Registered Nurse and a Registered Dietician.

We will cover:

  • Monitoring blood sugar.

  • Taking medication.

  • Problem solving.

  • Reducing risk.

  • Healthy coping.

  • Healthy eating.

  • Being active.

Classes are offered at various times on various days to best accommodate and can be provided individually if medically necessary.

Following your classes, you can schedule an individual visit with a Registered Dietician. This visit will provide a comprehensive nutrition assessment, as well as nutrition counseling for blood sugar control, carbohydrate counting, and meal planning.

Other Offerings

  • Class C (offered quarterly, this is a small group refresher class to ask questions, share successes, and review any educational needs).

  • Insulin pump and sensor program.

  • Diabetes in pregnancy education (gestational diabetes or pre-existing type 1/type 2 diabetes in pregnancy).

  • One on one education for non-English speaking patients (interpreter and ASL services available).

  • Free, community presentations on special diabetes topics (varies- call for scheduled events).

  • Our annual World Diabetes Day event.

  • Medical Nutrition Therapy (MNT) for other select topics, including weight loss, prediabetes, cholesterol improvement, blood pressure improvement, gastro-esophageal reflux disease, celiac disease, and more. Please call our office to inquire check insurance for coverage.

Pricing

Most insurance companies cover or partially cover diabetes education self-management Training (DSMT). Many will also cover Medical Nutrition Therapy (MNT) for people living with diabetes annually. Check with your insurance for coverage. You will be responsible for any fees the insurance does not cover. For questions regarding billing or payment plans, please call the St. Elizabeth Physicians Billing Department at (859) 344-5555. You must have a referral from your health care provider for diabetes education and/or MNT.
